Each year beginning in January the Town of West Springfield conducts the annual census. Every household receives the Annual Street Listing form to add their information into the census. Should your form get lost or damaged you can print a blank one from the link below. You can also use the blank form if you have changed your address and need to be added to the list of residents. Include all applicable information for everyone residing at your address.
College students and military service people can also be listed if they are maintaining a permanent address in West Springfield
